The Screamer's mission starts in the Velen map, from the Crow Perch mission board, remember to track the mission so you constantly see where you need to go to continue.

Talk to the man in the village and accept his offer, then ask the child behind the house about the screamer and head to the indicated place where, by investigating, you will find some traces of blood. Follow the blood, go to the cave and throw a bomb into the nest to lure the screamer out. Kill her and return to the man in the village to collect your reward.
